Receiving SLS Services

What Are Supported Living Services (SLS)?

Supported Living Services (SLS) are designed to support adults with developmental disabilities who want to live in their own home while receiving ongoing assistance tailored to their individual needs.

Unlike ILS, SLS provides a higher level of support and supervision. Services are highly individualized and can range from a few hours of support each week to 24-hour care depending on the person’s needs and goals.

SLS focuses on helping individuals:

  • Live safely in their own home

  • Build independence

  • Maintain stability

  • Participate in their community

  • Make personal choices

  • Develop life skills while receiving ongoing support

Services are funded through Regional Center of the East Bay (RCEB) and are based on assessed support needs, health and safety concerns, and long-term independence goals.

Overview of the SLS Process

Including Transitioning From ILS to SLS

  • Referral received from Regional Center of the East Bay (RCEB) for new SLS services or transition from ILS to SLS

  • Planning meeting with the client, family/conservator, RCEB, and our team

  • Review of increased support needs, safety concerns, goals, and living arrangements

  • Completion of intake paperwork, assessments, and service documents

  • Development of a personalized SLS support plan and staffing schedule

  • Coordination of apartment/home setup, routines, and community supports if needed

  • Carefully matched, trained, and background-cleared staff assigned

  • Staff complete client-specific training, safety procedures, and QSP onboarding

  • Transition period from ILS to SLS begins with additional supports and supervision added gradually as approved

  • SLS services officially begin with ongoing communication, supervision, and support coordination

while encouraging as much independence as possible.
