Supported Living Services (SLS)

Steps to Start SLS Services

1. Speak with Your Regional Center

​

Discuss SLS services with your service coordinator and explain:

• Current living situation

• Safety concerns

• Increased support needs

• Long-term independence goals

2. Regional Center Assessment & Planning

​

The regional center evaluates:

• Level of support needed

• Living goals

• Health & safety needs

• Hours of support required

​

This may include meetings with:

• Family members

• Conservators

• Existing providers

• Care teams

​

3. Select Bay Liberty as Your SLS Provider

​

Once Bay Liberty is chosen, we begin:

• Intake meetings

• Home visits

• Schedule planning

• Transition planning

• Staff preparation

​

We work closely with families to make the transition smooth and comfortable.

​

4. Develop the Support Plan

​

A personalized plan is created that may include:

• Daily routines

• Staffing schedules

• Community goals

• Health & safety plans

• Communication expectations

• Household support systems

​

5. Staffing & Training

​

Bay Liberty hires and trains staff specifically for each individual’s needs.

​

Our staff complete:

• Background checks

• CPR/First Aid certification

• Client-specific training

• Ongoing QSP documentation training

• Safety and emergency training

​

6. Coordination with IHSS

​

Many individuals receiving SLS may also qualify for In-Home Supportive Services (IHSS).

​

We help families understand:

• How SLS and IHSS work together

• Provider enrollment

• Scheduling support hours

• Care coordination

​

7. Services Begin

​

Once services are authorized:

• Staff schedules begin

• Goals and routines are implemented

• Ongoing communication starts with families and regional center teams

• Progress and support needs are monitored regularly
